4. Fax Copy of Hotel Reservations OR Letter of invitation from sponsor in Philippines
5. Copy of Travel itinerary

Consular Fees:

$35.00 Single Entry 3 Months
$65.00 Multiple Entry 6 Months
$95.00 Multiple Entry 1 Year

All applicants are expected to pay a visa fee for the visa using a credit or debit card. For a single entry visa with 3 months validity, the application fee is $30, but for multiple entry visas valid for 6 and 12 months, respectively, the visa fee is $60 and $90, respectively.

5. A Business Letter from U.S. Company address to “Consulate General of Philippines, explaining the purpose of the trip, guaranteeing financial support for you while in Philippines.
